Case management services are provided by health care professionals who work with patients to identify barriers that may prevent them from reaching their healthcare goals. Our case managers work closely with our medical and behavioral health providers while providing support and guidance to our patients.
Case managers strive to assist patients and families to navigate through the healthcare system. They provide advocacy for patients and collaborate with members of the healthcare team to find solutions in providing effective, quality, and efficient care. Case managers provide care coordination to facilitate appropriate delivery of health services and resource identification.
