Notice of Nondiscrimination:
All patients have the right to receive care at Tri Area Community Health (TACH) facilities without discrimination due to age, race, ethnicity, religion, culture, language, physical or mental disability, socioeconomic status, sex (sexual orientation, gender identity or expression and pregnancy), protected veteran status, marital status, genetic information, or any other protected characteristics under applicable federal, state or local law. TACH does not exclude people or treat them less favorably because of race, color, national origin, age, disability, or sex.
TACH provides free aids and services to people with disabilities to communicate effectively with us, such as:
- Qualified sign language interpreters
- Written information in other formats (large print, audio, accessible electronic formats, other formats).
We also provide free language assistance services to people whose primary language is not English, which may include:
- Qualified interpreters
- Information written in other languages.

You can file a grievance in person or by mail, fax, or email. If you need help filling a grievance, a front office staff member can assist you.
