Physician - Floyd

Tri-Area Community Health (https://triareahealth.org/) is a Federally Qualified Health Center headquartered in Laurel Fork, Virginia, with locations in Ferrum, Floyd, and Laurel Fork, Virginia. We offer integrated primary medical and behavioral health care in all locations and have pharmacies in Ferrum and Laurel Fork. All three sites are National Committee for Quality Assurance-certified Patient-Centered Medical Homes and medical professionals at all sites are eligible for National Health Service Corps Loan Repayment. For almost 40 years, Tri-Area has provided care to underserved individuals in Southwest Virginia.

We are seeking a Board-certified / Board-eligible Family Medicine Physician to work 2 days a week (Wednesdays & Fridays) and join the Floyd Clinic.

This position involves providing complex direct medical care of patients, including older adults, individuals with two or more chronic medical conditions, and people experiencing difficult social determinants of health. The work is performed independently with oversight provided by the Medical Director, on site. The Physician must operate in accordance with the terms and conditions of the various contracts and grants providing for the funding of programs and projects.

Pay will be based on education and experience.

Graduation from a medical school approved by the Council on Medical Education and Hospitals of the American Medical Association or the Committee on Hospitals of the Bureau of Professional Education of the American Osteopathic Association. New graduates are welcome to apply.

Physician or Nurse Practitioner - Baywood

JOB SUMMARY:

Tri-Area Community Health (https://triareahealth.org/) is a Federally Qualified Health Center headquartered in Laurel Fork, Virginia, with full-service locations currently open in Ferrum, Floyd, Grayson Highlands (Troutdale), and Laurel Fork, Virginia. For almost 40 years, Tri-Area has provided care to underserved individuals in Southwest Virginia. We offer integrated primary medical and behavioral health care in all these clinic locations. We also have pharmacies that are open in Ferrum and Laurel Fork. Our three large sites are National Committee for Quality Assurance-certified Patient-Centered Medical Homes and medical professionals at all sites are eligible for National Health Service Corps Loan Repayment (HPSA score of 19).

We are excited to be opening a new single-medical provider clinic and single-pharmacist pharmacy in Baywood, Virginia (located in Grayson County, near the city of Galax). This clinic and pharmacy will be co-located in a building that also will house a technology training center and the plan is to collaborate with that training center on utilizing innovative technology to provide care to patients and service to pharmacy customers.

POSITION:

We are seeking a Board-certified / Board-eligible Family Medicine Physician or a Family Nurse Practitioner to work full-time (5 days - 40 hours per week) in our new Baywood clinic.

This position involves providing complex direct medical care of patients, including older adults, individuals with two or more chronic medical conditions, and people experiencing difficulty with social determinants of health. The work is performed independently with oversight provided by the Medical Director. The Physician / Nurse Practitioner must operate in accordance with the terms and conditions of the various contracts and grants providing the funding for Tri-Area’s programs and projects. In addition, the Baywood site is envisioned to be an “innovation hub” for the organization and therefore all staff (especially the Physician / Family Nurse Practitioner and the Pharmacist) must be willing and able to explore options, try new processes and technologies, and help lead the organization into the future. We need people who are forward-thinking, aware of the emerging changes in the healthcare environment, and excited about seeking out new opportunities.

Pay will be based on education and experience.

REQUIREMENTS:

Graduation from a medical school approved by the Council on Medical Education and Hospitals of the American Medical Association or the Committee on Hospitals of the Bureau of Professional Education of the American Osteopathic Association. New medical school graduates are welcome to apply. Nurse Practitioners must be licensed to practice as a Family Nurse Practitioner in the Commonwealth of Virginia.

Duties include retail dispensing functions including medication counseling and vaccinations. The ideal candidate will have great customer service skills, attention to accuracy, and enthusiasm for assisting patients and clinic providers. Experience with 340b is a plus. Applicant must have a valid Virginia pharmacist’s license, BS in Pharmacy or Pharm.D. degree. Applicant bilingual in Spanish preferred.
