Tri-Area Community Health’s Transportation Service Team is committed to providing safe, quality, and comfortable transportation throughout our area of Southwestern Virginia. This service is for patients who have no other means of getting to and from their Tri-Area Community Health medical and behavioral health appointments.

Medical transportation services are available to patients at Tri-Area Community Health sites in Carroll, Floyd, and Franklin Counties who live in the designated area near the health center where they are seen.

Our Transportation Guidelines

Before arranging transportation with Tri-Area Community Health, we ask that you please review the following guidelines. In order to maintain a schedule and provide this service to all patients throughout our area, patients and passengers must understand and adhere to these guidelines. If you have any questions, please don’t hesitate to contact us.

1. Patients will be picked up and dropped off at their address on file unless prior approval is made with the Transportation Coordinator.

2. Minors must be accompanied by a parent, legal guardian, or an authorized caregiver indicated on the patient’s registration form. Patients aged eight or younger must ride in a car/booster seat at all times. Please let us know when scheduling a ride if you will need a car seat.

3. The patient must inform the Transportation Coordinator how many guests will be traveling with them. No more than two guests are allowed to ride without prior approval.

4. All patients and guardians of minor children must show a government-issued ID before boarding the vehicle.

5. Inform the Transportation Coordinator if you need wheelchair accessible transportation.

6. Patients and passengers must be ready at their designated pick-up time.

Available to New and Existing Patients at Three of Our Clinics

Whether you’re a new or existing patient at Tri-Area Community Health, our medical transportation service is available to drive you between your home and the nearest clinic. We provide this service to patients that live within the designated area surrounding our Southwestern Virginia health centers located in Ferrum, Floyd, and Laurel Fork. To schedule your pick-up, simply contact us before your appointment to get set up. Please review our transportation guidelines (above) and let us know if you have any questions.

If you’re joining Tri-Area Community Health as a new patient, please be sure to bring your medical records and prescription information for your first appointment. Because of the nature and scheduling of our health transportation vehicles, we may be unable to turn back if you forget to bring these items.

Call our Transportation Coordinator at 276-398-2292 or (833) 857-2981 before your appointment.

Our vehicles are assigned to different sites on different days. Every effort will be made to accommodate your appointment date and time. The Transportation Coordinator will contact you before your scheduled appointment to arrange pick-up times. Please call the office to check availability for same-day appointments.
